Bakersfield PACE is a Program of All-inclusive Care for the Elderly (PACE), a long-term comprehensive health care program that enables older adults to live at home in their communities for as long as medically and socially possible. Bakersfield PACE is committed to helping older adults maintain their independence, dignity and quality of life. Bakersfield PACE is a Health Plan which provides all medical care (24-hours-a-day), a PACE Day Center, home care services and transportation.
If you are eligible for Medi-Cal and Medicare, all Bakersfield PACE services are covered at no additional cost. For Medi-Cal beneficiaries who have a “share-of-cost” you will continue to be responsible for the individual share-of-cost. If you don’t qualify for Medi-Cal you will be charged a monthly premium to cover the long-term care portion of the PACE benefit and a premium for Medicare Part D drugs. There is no deductible or copayment for any drugs, services, or care approved by the Bakersfield PACE health care team. Bakersfield PACE also accepts Medi-Cal-only and private-pay individuals.
The PACE Program creatively coordinates the care of each participant enrolled in the program based on his or her individual needs. An interdisciplinary team develops a Plan of Care with the goal of enabling older individuals to remain living safely in their community.
For those who qualify to participate in the Bakersfield PACE program, services include but are not limited to: Physician, specialist, and nursing care; prescription drugs and durable medical equipment; dental, eye and foot care; hospital, emergency and nursing home care; rehabilitative and recreational therapies. • Are 55 years or older
• Are a resident of Kern or Tulare County
• Are able to live in the community safely
• Meet the level of care requirements as determined by the California Department of Health Care Services

Transportation is provided by Bakersfield PACE! One of our specially equipped vans picks you up at home and brings you to Bakersfield PACE for the day. You may get a medical clinic check-up, physical and rehabilitative therapy, depending on your needs. You will get hot meals, plus social and recreational activities with other participants at our PACE Center.
Those who are eligible will receive in-home care services (these services must be necessary and you must be approved by our interdisciplinary team to receive them), including but not limited to: Personal care (bathing, grooming, dressing), light housekeeping.
In order to participate in the PACE program you will be required to be cared for by our primary care provider. PACE acknowledges that you may have a long term relationship with your own physician and we will work closely with him/her to ensure a seamless transition to our care.
Bakersfield PACE participants agree to receive all health care services exclusively from Bakersfield PACE interdisciplinary team made up of a physician, clinic and home-care nurses, social worker, physical, occupational, speech and recreational therapists, health care aides and transportation staff.
First we schedule a meeting/presentation with you and your caregiver(s) to tell you more about Bakersfield PACE program, this meeting can be at your home or the Bakersfield PACE center.
The Bakersfield PACE enrollment process can take a couple of weeks depending on the availability of the PACE team. Our goal is to work with you and get through the process at your convenience.
When you become member you will be assigned a primary care provider who is part of our interdisciplinary team. Our primary care provider will work with you and other members of our team to coordinate all services a participant receives. Our primary care providers are located in the clinic at the Bakersfield PACE center so participants have excellent access to them.
