To enable frail, underserved, and multiethnic senior communities to enjoy an improved quality of life and to age at home with dignity by providing high quality health and human services through an innovative world-class delivery system.
To be the leading geriatric healthcare provider in Kern and Tulare County, providing innovative and comprehensive services that improve the quality of life of the communities we serve.
With the increasing demand for needs to service the elderly population of Central California, Innovative Integrated Health (IIH) opened its doors to Kern and Tulare County and the surrounding community on January 1, 2020 with the grand opening of Bakersfield PACE. At the heart of this launch is a commitment to bring forth all services to keep the seniors of our program living healthy.
Six years ago, IIH started its first PACE program – Fresno PACE with the vision to be the leading geriatric healthcare provider in Fresno County and so it is with great pleasure that we can now bring the same services to Kern and Tulare County. We’re proud to be the only service of this kind in this valley and look forward to a collaborative partnership with all medical care facilities to reshape how care is driven.
The over 25,000 sq ft Adult Day Health Center is home to older adults and their caregivers. The newly remodeled facility provides world-class medical care to members of any and all backgrounds. The one thing that differentiates IIH’s PACE programs from all others across the nation is that we are a truly diverse make-up of both professionals and participants. We believe that when people of all walks of life come together, we can learn, we can build relationships, we can provide care.
Bakersfield PACE serves seniors who:
Bakersfield PACE accepts Medicare and/or Medi-Cal. Bakersfield PACE members have no out-of-pocket costs for healthcare services, prescriptions or any of our services.
